[a] This Form must be completed in full and returned to the Entry Secretary as soon as possible.
[b] Details on this Registration Form will be used for Race Meetings throughout the year. In the event of any change, please complete a new Registration Form or notify the Entry Secretary, in writing.
[c] A minimum of FIVE frequency choices must be given and be able to be provided at all meetings. Please provide further frequency choices to aid the formation of heats. If you do not bring the same five frequencies to an event, you may forfeit your place.
(d) Personal transponders are compulsory and it is the responsibility of the driver to ensure that the transponder used corresponds to that detailed on this form (or on any subsequent written notification).
